OUTPATIENT MENTAL HEALTH CENTER (OMHC)

  • Medication management

  • Co-occurring substance abuse screening assessments

  • Comprehensive psychiatric assessments for minors

  • Somatic evaluations

  • Supportive housing referrals and placements

  • Medical records requests

  • Individual treatment plan & treatment plan reviews

  • Mental health treatment (individual, group and family therapy)

  • Substance abuse treatment (counseling, IOP & detox referral)

  • On-call crisis interventions (for emergencies call 911)

  • Referral services

  • Case coordination and assistance with entitlements

  • Linkage with higher levels of care

  • Education regarding medication

PSYCHIATRIC REHABILITATION PROGRAM (PRP-A/M)

  • Rehabilitation assessments and screenings

  • Community integration and supportive

  • Ex-offender reentry services

  • Support to rebuild rehabilitation & recovery skills such as:

    • Self-care skills

    • Social skills,

    • Independent living skills,

    • Activities that support the individual's cultural interests

  • Medication administration and monitoring​

  • Health promotion and training

  • Promotion of individual wellness self management & recovery

  • On-call crisis response

  • Linkage with individuals in psychiatric institutions
